The crash happened around 8:00 a.m. September 11, on Loop 20 near Lake Casa Blanco.

As per the report, a semi-truck crashed head-on into three other vehicles.

Jesus Tapia, Jr., 30, was killed in the crash.

Rosario Cabello sustained injuries in the crash.
