The deadly accident comes about four days after the FAA notice warned that some lights on the tower were inoperable.

Houston, TX – Four people, including a child, are dead after a helicopter, a privately owned Robinson R44, struck a communications tower and erupted into a fireball in Houston on Sunday night.

The tragic incident happened at about 7:54 p.m. on October 20 near Houston’s Second Ward.

An R44 helicopter reportedly struck a radio tower after taking off from Ellington Field about 15 miles away.

Four people including a child on board the helicopter were killed in the accident.

Authorities say the helicopter was used for private tours.

The Federal Aviation Administration (FAA) had published a notice four days before warning that some of the lights on the tower, located near the intersection of Engelke and Ennis streets, were not functional.

The FAA and National Transportation Safety Board (NTSB) are investigating the crash.
